Panitumumab Plus FOLFOXIRI in RAS Wild-Type Metastatic Colorectal Cancer

On October 10, 2019, Dominik P. Modest published in the Journal of Clinical Oncology the VOLFI/AIO KRK0109 Phase 2 trial results: "Efficacy and safety of anti-EGFR antibody panitumumab (Vectibix) plus FOLFOXIRI chemotherapy in treatment-naive RAS wild-type metastatic colorectal cancer patients."
Trial Design
The VOLFI/AIO KRK0109 Phase 2 trial was an open-label study in 96 treatment-naive patients with RAS wild-type metastatic colorectal cancer, randomized 2:1 to:
- Panitumumab + FOLFOXIRI group (N=63)
- FOLFOXIRI group (N=33)
Primary endpoint: ORR. Secondary endpoints: secondary resection rate, OS, and PFS.
Efficacy Results
ORR (primary endpoint):
- Panitumumab + FOLFOXIRI group: 87.3% / FOLFOXIRI group: 60.6%
- The panitumumab + FOLFOXIRI group exceeded the trial's predefined threshold (75%) and was significantly higher than FOLFOXIRI alone
Secondary resection rate:
- Panitumumab + FOLFOXIRI group: 33.3% / FOLFOXIRI group: 12.1% — improved in the combination group
Both groups had similar PFS. OS was also improved in the panitumumab + FOLFOXIRI group.
Conclusions
Based on the VOLFI/AIO KRK0109 trial results, Dominik P. Modest concluded that anti-EGFR antibody panitumumab plus FOLFOXIRI improved both objective response rate and resection rate compared to FOLFOXIRI alone in treatment-naive RAS wild-type metastatic colorectal cancer patients, making it a promising new treatment option for this population.
