Required Documents

Valid passport (at least 6 months remaining)

Visa application form

2-inch white background photo (taken within 6 months)

Certificate of scheduled consultation issued by the medical institution

Guarantor-related documents when required, confirmed through a partner travel agency or administrative scrivener

Proof of ability to cover expenses (bank statement, etc.)

Identity or residence documents, depending on nationality and application location

Certificate of Eligibility (when staying longer than 90 days)

Treatment schedule (issued by the Japanese physician)

Companion Rules

Not limited to family members; non-relatives may accompany when necessary

Up to 1–2 companion visas may be applied for
