Niraparib Plus Bevacizumab in Platinum-Sensitive Recurrent Ovarian Cancer

On August 29, 2019, Mansoor Raza Mirza published in the medical journal The Lancet a comparative efficacy study: "PARP inhibitor niraparib plus VEGF antibody bevacizumab in platinum-sensitive recurrent ovarian cancer — NSGO-AVANOVA2/ENGOT-ov24 Phase 1/2 trial results."
Trial Design
This Phase 1/2 trial enrolled 97 patients with platinum-sensitive recurrent ovarian cancer in a randomized controlled design:
- Niraparib + bevacizumab group: N=48
- Niraparib monotherapy group: N=49
Primary endpoint: progression-free survival (PFS). Secondary endpoint: disease control rate.
Efficacy Results
At a median follow-up of 16.9 months:
- Median PFS: Niraparib + bevacizumab: 11.9 months / Niraparib alone: 5.5 months
- The niraparib + bevacizumab group showed a 65% reduction in disease progression risk, with a hazard ratio of 0.35 — a statistically significant improvement.
Safety Profile
Grade 3 or higher adverse event rates:
- Niraparib + bevacizumab: 65% (N=31) / Niraparib alone: 45% (N=22)
Most common Grade 3 adverse events:
- Anemia: Niraparib + bevacizumab 15% / Niraparib alone 18%
- Thrombocytopenia: Niraparib + bevacizumab 10% / Niraparib alone 12%
- Hypertension: Niraparib + bevacizumab 21% / Niraparib alone 0%
No patients in either group died due to adverse events.
Conclusions
Based on the NSGO-AVANOVA2/ENGOT-ov24 results, Mansoor Raza Mirza concluded that PARP inhibitor niraparib plus VEGF antibody bevacizumab demonstrated significant improvement in progression-free survival in patients with platinum-sensitive recurrent ovarian cancer across various chemotherapy regimens.
