At ASCO 2023 held in Chicago, USA from June 2–6, 2023, Nagla Fawzy Abdel Karim et al. from Inova Schar Cancer Institute presented the efficacy and safety results from the SWOG S1929 Phase 2 clinical trial evaluating anti-PD-L1 antibody monotherapy and anti-PD-L1 antibody + Talazoparib as maintenance therapy in SLFN11-positive extensive-stage small cell lung cancer.

The SWOG S1929 Phase 2 clinical trial randomized patients with SLFN11-positive extensive-stage small cell lung cancer 1:1 into either a anti-PD-L1 antibody group or a anti-PD-L1 antibody + Talazoparib group, with the primary endpoint of progression-free survival and secondary endpoints of objective response rate, overall survival, and safety.

In this trial, the median age of the 106 patients was 67 years (range 45–84), 48% were female (N=51), 89% were White (N=94), and 96% had a Performance Status score of 0–1 (N=102).

The median progression-free survival for the primary endpoint was 2.8 months in the anti-PD-L1 antibody group vs. 4.2 months in the anti-PD-L1 antibody + Talazoparib group. The median overall survival for the secondary endpoint was 8.5 months in the anti-PD-L1 antibody group vs. 9.4 months in the anti-PD-L1 antibody + Talazoparib group. The objective response rates were 16% for the anti-PD-L1 antibody group vs. 12% for the anti-PD-L1 antibody + Talazoparib group.

Regarding safety, the incidence of grade 3 or higher non-hematologic adverse events was 13% in the anti-PD-L1 antibody group vs. 15% in the anti-PD-L1 antibody + Talazoparib group. The incidence of grade 3 or higher hematologic adverse events was 4% in the anti-PD-L1 antibody group vs. 0% in the anti-PD-L1 antibody + Talazoparib group. No grade 5 or higher adverse events were confirmed in either group.

Based on the results of the SWOG S1929 trial, Nagla Fawzy Abdel Karim et al. concluded: In SLFN11-positive extensive-stage small cell lung cancer, anti-PD-L1 antibody + Talazoparib as maintenance therapy improved progression-free survival compared to anti-PD-L1 antibody monotherapy, but was associated with an increased rate of hematologic adverse events.
