On May 23, 2022, Benjamin J. Solomon et al. from Peter MacCallum Cancer Centre published results in the Journal of Clinical Oncology on the post-hoc analysis of the CROWN Phase III clinical trial evaluating the efficacy and safety of ALK inhibitor ALK-targeted drug as first-line treatment for ALK fusion gene mutation-positive advanced non-small cell lung cancer patients.
The CROWN Phase III clinical trial randomized ALK fusion gene mutation-positive advanced non-small cell lung cancer patients in a 1:1 ratio into the ALK-targeted drug group (100 mg once daily) and the Crizotinib group (250 mg twice daily). The primary endpoint was progression-free survival.
The trial results showed that the 12-month progression-free survival rate in patients with brain metastases was 78% for the ALK-targeted drug group / 22% for the Crizotinib group. In patients without brain metastases, the 12-month progression-free survival rate was 78% for the ALK-targeted drug group / 45% for the Crizotinib group. The 12-month incidence of central nervous system disease progression in patients with brain metastases was 7% for the ALK-targeted drug group / 72% for the Crizotinib group. In patients without brain metastases, the 12-month incidence of central nervous system disease progression was 1% for the ALK-targeted drug group / 18% for the Crizotinib group.
Regarding safety, the adverse event rate in the ALK-targeted drug group was 35%, with most being grade 1 adverse events.
Based on the post-hoc analysis of the CROWN trial, Benjamin J. Solomon et al. stated that ALK inhibitor ALK-targeted drug as first-line treatment for ALK fusion gene mutation-positive advanced non-small cell lung cancer patients improved progression-free survival and suppressed central nervous system disease progression.
