From May 29 to 31, 2020, Egbert F. Smit presented at the ASCO 2020 Virtual Meeting the results on the efficacy and safety of anti-HER2 antibody drug anti-HER2 antibody-drug conjugate monotherapy in HER2-positive metastatic non-small cell lung cancer patients from the DESTINY-Lung01 Phase II clinical trial.
The DESTINY-Lung01 Phase II clinical trial administered anti-HER2 antibody-drug conjugate monotherapy at 6.4mg/kg in 3-week cycles to HER2-positive metastatic non-small cell lung cancer patients. The primary endpoint was objective response rate, with secondary endpoints including duration of response, progression-free survival, and safety.
Patient characteristics in this trial (42 patients) were as follows: female 64.3%, median age 63.0 years, under 65 years old 59.5%, patients with central nervous system metastases 45.2%, ECOG score 0 at 23.8%, ECOG score 1 at 76.2%, median number of prior treatments was 2, and prior medication history included: platinum-based agents 54.8%, anti-PD-1/PD-L1 antibody drugs 54.8%.
At the data cutoff in November 2019 (median treatment duration 7.75 months), the results were as follows: primary endpoint objective response rate was 61.9%, secondary endpoints showed median duration of response not yet reached, median progression-free survival of 14.0 months, and disease control rate of 90.5%.
Regarding safety, the overall adverse event rate was 100%, grade 3 or higher adverse event rate was 64.3%, treatment interruption rate due to adverse events was 59.5%, dose reduction rate was 38.1%, treatment discontinuation rate was 23.8%, and interstitial lung disease was 11.9% (N=5 patients).
Based on the DESTINY-Lung01 trial results, Egbert F. Smit concluded that anti-HER2 antibody drug anti-HER2 antibody-drug conjugate monotherapy demonstrated a high objective response rate in HER2-positive metastatic non-small cell lung cancer patients, with adverse events consistent with those observed in other clinical trials.
