On November 3, 2022, Melissa L. Johnson et al. from Sarah Cannon Research Institute published in the medical journal Journal of Clinical Oncology the efficacy and safety results of anti-PD-L1 antibody anti-PD-L1 antibody + anti-CTLA-4 antibody Tremelimumab + chemotherapy as first-line treatment for metastatic non-small cell lung cancer patients in the POSEIDON Phase III clinical trial.
The POSEIDON Phase III clinical trial was an international multi-center trial that randomized metastatic non-small cell lung cancer patients (N=1,013) in a 1:1:1 ratio into the anti-PD-L1 antibody + Tremelimumab + chemotherapy group, the anti-PD-L1 antibody + chemotherapy group, and the chemotherapy group. The primary endpoints were progression-free survival and overall survival for the anti-PD-L1 antibody + chemotherapy group, with secondary endpoints being progression-free survival and overall survival for the anti-PD-L1 antibody + Tremelimumab + chemotherapy group.
The trial results showed that the primary endpoint median progression-free survival for the anti-PD-L1 antibody + chemotherapy group was 5.5 months with a median overall survival of 13.3 months. The secondary endpoint median progression-free survival for the anti-PD-L1 antibody + Tremelimumab + chemotherapy group was 6.2 months with a median overall survival of 14.0 months.
Regarding safety, grade 3-4 adverse event rates were 51.8% for the anti-PD-L1 antibody + Tremelimumab + chemotherapy group, 44.6% for the anti-PD-L1 antibody + chemotherapy group, and 44.4% for the chemotherapy group. Treatment discontinuation due to adverse events occurred in 15.5% of the anti-PD-L1 antibody + Tremelimumab + chemotherapy group, 14.1% of the anti-PD-L1 antibody + chemotherapy group, and 9.9% of the chemotherapy group.
Based on the POSEIDON trial results, Melissa L. Johnson et al. stated that anti-PD-L1 antibody + Tremelimumab + chemotherapy as first-line treatment for metastatic non-small cell lung cancer patients showed improvement in progression-free survival and overall survival compared to chemotherapy alone. anti-PD-L1 antibody + chemotherapy also showed improved progression-free survival compared to chemotherapy, and anti-PD-L1 antibody may become a new first-line treatment option for metastatic non-small cell lung cancer.
