（淋巴瘤）VENCLEXTA＋Obinutuzumab＋bendamustine有效？

2021年12月11日～13日，University of Virginia的Craig A. Portell等人，在第63屆美國血液學會上發表了，高腫瘤負荷的濾泡性淋巴瘤患者使用BCL-2抑制劑VENCLEXTA＋抗CD20抗體藥Obinutuzumab＋bendamustine作為第一線治療，在PrECOG 0403第二期臨床試驗的有效性及安全性結果。

PrECOG 0403第二期臨床試驗是以28天為一個治療週期，向高腫瘤負荷的濾泡性淋巴瘤患者（N＝56人）使用VENCLEXTA（第1~10天，每日一次800mg）＋Obinutuzumab（每日一次1000mg，第一個治療週期為第1天100mg，第2天900mg，第8、15天1000mg）＋bendamustine（第1、2天 90mg/m2）作為第一線治療，主要評價項目為完全緩解率。

本次試驗的患者年齡中位數為為 62 歲（33-79 歲）、男性63% (N = 35)，女性38% (N = 21) 、癌症II期4%，III 期29%，IV期 68%，本次試驗的結果如下。

主要評價項目完全緩解率為 73.2%，隨訪中位數20.9個月時的次要評價項目2年總生存率為94.4％、2年無惡化生存率為85.8％。

在安全性方面，有20％以上的患者發生副作用，分別的發生率為噁心82.1%、疲勞60.7%、嘔吐46.4%、腹瀉42.9%、血小板減少症41.1%、中性粒細胞減少 37.5%、頭痛 28.6%、食慾不振 26.8%及貧血 21.4%。等級三以上的副作用發生率為83.9%，主要有中性粒細胞減少症16.1%、腫瘤溶解症候群14.3%和血小板減少症14.3%。

根據PrECOG 0403試驗的結果，Craig A. Portell等人表示，高腫瘤負荷的濾泡性淋巴瘤患者使用BCL-2抑制劑VENCLEXTA＋抗CD20抗體藥Obinutuzumab＋bendamustine作為第一線治療，主要評價項目完全緩解率達到基準值，無惡化生存期間有改善的可能性，在安全性方面等級三以上的副作用發生率為83.9％，需要繼續追蹤調查。
