On January 19, 2022, Vicky Makker of Memorial Sloan Kettering Cancer Center published results in The New England Journal of Medicine from the Study 309–KEYNOTE-775 Phase III clinical trial evaluating lenvatinib plus the anti-PD-1 antibody Keytruda (pembrolizumab) in advanced endometrial cancer patients with prior platinum-based chemotherapy.
The Study 309–KEYNOTE-775 Phase III trial randomized previously platinum-treated advanced endometrial cancer patients (N=827) in a 1:1 ratio to the lenvatinib arm (lenvatinib 20 mg once daily + Keytruda 200 mg every 3 weeks, N=411) or the chemotherapy arm (N=416). Primary endpoints were progression-free survival (PFS) and overall survival (OS).
Of the 827 patients, 697 were pMMR and 130 were dMMR.
Progression-Free Survival (Primary Endpoint): Median PFS was 7.2 months in the lenvatinib arm versus 3.8 months in the chemotherapy arm, representing a 44% reduction in the risk of progression or death in the lenvatinib arm. Among pMMR patients, median PFS was 6.6 months versus 3.8 months, with a 40% reduction in risk.
Overall Survival (Primary Endpoint): Median OS was 18.3 months in the lenvatinib arm versus 11.4 months in the chemotherapy arm, representing a 38% reduction in the risk of death. Among pMMR patients, median OS was 17.4 months versus 12.0 months, with a 32% reduction in risk.
Safety: Grade ≥3 adverse events occurred in 88.9% of lenvatinib arm patients versus 72.7% in the chemotherapy arm.
Based on the Study 309–KEYNOTE-775 results, Vicky Makker and colleagues concluded that lenvatinib plus Keytruda demonstrated significantly improved PFS and OS compared with chemotherapy in previously platinum-treated advanced endometrial cancer patients.
