On January 9, 2023, Kaysia Ludford and colleagues from The University of Texas MD Anderson Cancer Center published results of a Phase 2 clinical trial (NCT04082572) in the Journal of Clinical Oncology, evaluating the efficacy and safety of Keytruda as neoadjuvant therapy in patients with advanced metastatic solid tumors with MSI-High and dMMR status.

This Phase 2 clinical trial was conducted as an open-label trial at independent facilities, administering Keytruda (600mg per 3-week cycle, maximum 6 months treatment duration) as neoadjuvant therapy to patients with advanced metastatic solid tumors with MSI-High and dMMR status. Following surgery, patients continued treatment for up to one year based on individual choice. The primary endpoints were pathologic complete response rate and safety, while secondary endpoints included organ preservation rate in patients not undergoing surgery.

Results from 35 enrolled patients (27 with colorectal cancer, 8 with non-colorectal cancer) were as follows: among 33 patients evaluable for efficacy, the objective response rate was 82%, and the primary endpoint of pathologic complete response rate (in 17 patients who underwent surgery) was 65%.

Regarding safety, no new adverse events were identified in this trial, consistent with the current safety profile of Keytruda from other clinical trials.

Based on Phase 2 clinical trial results, Kaysia Ludford and colleagues concluded: Patients with advanced metastatic solid tumors with MSI-High and dMMR status treated with Keytruda as neoadjuvant therapy demonstrated good tolerability to adverse events and favorable pathologic complete response rates, offering patients an organ-preservation option.
