On August 3, 2022, Thierry André et al. of Saint-Antoine Hospital published in the medical journal Journal of Clinical Oncology the efficacy and safety results of nivolumab (Opdivo) + ipilimumab (Yervoy) as neoadjuvant chemotherapy followed by nivolumab monotherapy in patients with dMMR and MSI-H locally advanced gastric cancer and gastroesophageal junction adenocarcinoma in the GERCOR NEONIPIGA Phase 2 clinical trial.
The GERCOR NEONIPIGA Phase 2 clinical trial administered nivolumab (2-week cycle, 240 mg per dose, 6 cycles total) + ipilimumab (6-week cycle, 1 mg/kg per dose, 2 cycles total) as preoperative therapy to dMMR and MSI-H locally advanced gastric cancer and gastroesophageal junction adenocarcinoma patients (N=32), followed by postoperative nivolumab (4-week cycle, 480 mg per dose). The primary endpoint was complete response rate.
In this trial, the median patient age was 65.5 years. Clinical staging was cT2-T3N0 (N=9), cT2-T3N1 (N=22), and cT3N1M1 (N=1). Results at a median follow-up of 14.9 months were as follows.
The incidence of Grade 3–4 adverse events during preoperative treatment was 19% (N=6). Among the 29 patients who underwent surgery after treatment, all 29 achieved R0 resection, with a complete response rate of 58.6% (N=17).
Based on the GERCOR NEONIPIGA trial results, Thierry André et al. concluded that nivolumab + ipilimumab as neoadjuvant chemotherapy had good tolerability and demonstrated a high complete response rate.
