Keytruda and Chemotherapy in Advanced Endometrial Cancer

On June 3, 2023, Dr. Ramez N. Eskander and colleagues from the University of California San Diego published the results of the Phase 3 NRG-GY018 trial in The New England Journal of Medicine. The study evaluated the addition of Keytruda (pembrolizumab) to standard first-line chemotherapy for patients with recurrent or advanced endometrial cancer.
Study Design: The NRG-GY018 trial enrolled 816 patients with Stage III, IVA, IVB, or recurrent endometrial cancer. Participants were randomized 1:1 into two groups:
- Keytruda Group: Keytruda plus chemotherapy (up to 6 cycles every three weeks), followed by Keytruda monotherapy as maintenance (up to 14 cycles every six weeks).
- Placebo Group: Standard chemotherapy plus placebo.
The primary endpoint was progression-free survival (PFS), stratified by mismatch-repair-deficient (dMMR) or mismatch-repair-proficient (pMMR) status.
Clinical Outcomes: The trial results showed significant improvements in PFS for the Keytruda group:
- dMMR Patients: The 12-month PFS rate was 74% in the Keytruda group compared to 38% in the placebo group.
- pMMR Patients: The median PFS was 13.1 months in the Keytruda group versus 8.7 months in the placebo group.
Conclusion: Based on the results of the NRG-GY018 trial, Dr. Ramez N. Eskander and the research team concluded that adding Keytruda to first-line chemotherapy significantly improves progression-free survival in patients with recurrent or advanced endometrial cancer.
