On September 18, 2021, AstraZeneca announced the results of the DESTINY-Gastric02 Phase 2 clinical trial, evaluating Enhertu (Trastuzumab deruxtecan) as a second-line therapy for patients with HER2-positive advanced gastric cancer. The findings were presented at the ESMO Congress 2021.
Study Design: DESTINY-Gastric02 Trial
DESTINY-Gastric02 is an international, multicenter, open-label, single-arm Phase 2 trial. It enrolled patients with HER2-positive advanced gastric or gastroesophageal junction (GEJ) cancer who had progressed on or after a trastuzumab-containing regimen.
Patients received Enhertu at a dose of 6.4 mg/kg every three weeks. The primary endpoint was objective response rate (ORR).
Key Efficacy Results (Median Follow-up 5.7 Months)
- Objective Response Rate (ORR): 38.0%
- Complete Response (CR): 3.8%
- Partial Response (PR): 34.2%
- Disease Control Rate (DCR): 81.0%
- Median Duration of Response (DOR): 8.1 months
- Median Progression-Free Survival (PFS): 5.5 months
Safety Profile
The safety profile was consistent with previous trials of Enhertu. The most common Grade 3 or higher adverse events included:
- Anemia: 7.6%
- Neutropenia: 7.6%
- Interstitial Lung Disease (ILD): 7.6% (N=6 patients)
- Nausea and Fatigue: 3.8% each
Most adverse events were Grade 1 or 2.
Conclusion
Dr. Eric Van Cutsem from University Hospitals Leuven concluded that Enhertu demonstrates a clinically meaningful response in patients with HER2-positive advanced gastric cancer who have failed prior treatment, establishing its role as a potent second-line option.
