COX-2 Expression as Prognostic Biomarker

On January 31, 2019, the Journal of Clinical Oncology published exploratory analysis from the REMAGUS02 Phase III trial examining the relationship between COX-2 inhibition via celecoxib and COX-2 gene expression (PTGS2) in locally advanced HER2-negative breast cancer patients receiving neoadjuvant chemotherapy.
Trial Design
Among 156 locally advanced, COX-2-expressing HER2-negative breast cancer patients:
Group 1: Epirubicin plus cyclophosphamide (standard regimen) followed by Docetaxel plus Celecoxib
Group 2: Epirubicin plus cyclophosphamide followed by Docetaxel monotherapy
Stratification: age, menopausal status, TNM classification, hormone receptor status
Key Findings
COX-2 Low-Expression Subgroup:
- Concurrent docetaxel plus celecoxib resulted in reduced event-free survival and overall survival
COX-2 High-Expression Subgroup:
- No significant impact of celecoxib addition
Clinical Significance
Results demonstrate that COX-2 expression status serves as a predictive biomarker for celecoxib efficacy in breast cancer neoadjuvant therapy. Patient stratification by COX-2 expression may optimize therapeutic outcomes and minimize unnecessary toxicity.
