On September 19, 2022, Alexander Drilon and colleagues from Memorial Sloan Kettering Cancer Center and Weill Cornell Medical Center published the results of the Phase 1/2 LIBRETTO-001 trial in the "Journal of Clinical Oncology." The study evaluated the efficacy and safety of Retevmo (selpercatinib) in patients with RET fusion-positive non-small cell lung cancer (NSCLC).

LIBRETTO-001 is a single-arm, open-label Phase 1/2 trial. A total of 316 patients with RET fusion-positive NSCLC were treated with Retevmo. The primary endpoint was objective response rate (ORR), and secondary endpoints included duration of response (DOR), progression-free survival (PFS), overall survival (OS), and safety. The study included 69 treatment-naive patients and 247 patients who had previously received platinum-based chemotherapy.

For treatment-naive patients, the primary endpoint ORR was 84%, with a 6% complete response (CR) rate. Secondary endpoints included a median DOR of 20.2 months and a median PFS of 22.0 months.

For patients previously treated with platinum-based chemotherapy, the primary endpoint ORR was 61%, with a 7% CR rate. Secondary endpoints included a median DOR of 28.6 months and a median PFS of 24.9 months.

In patients with central nervous system (CNS) metastases (N=26), the intracranial ORR was 85%, with a 27% CR rate.

Regarding safety, the adverse event profile was consistent with previously reported data, and no new safety signals were identified.

Based on the LIBRETTO-001 results, Alexander Drilon stated that Retevmo demonstrates significant anti-tumor activity in patients with RET fusion-positive NSCLC, regardless of prior treatment, including those with intracranial metastases.
