Is Opdivo + Chemotherapy Effective for Lung Cancer?

On August 10, 2023, Mariano Provencio and colleagues from Puerta de Hierro University published the results of a Phase 2 clinical trial in "The New England Journal of Medicine." The study evaluated the efficacy and safety of neoadjuvant Opdivo plus platinum-based chemotherapy in patients with Stage IIIA/IIIB non-small cell lung cancer (NSCLC).

This open-label Phase 2 trial randomized patients with Stage IIIA/IIIB NSCLC (N=86) to receive either Opdivo + chemotherapy (N=57) or chemotherapy alone (N=29). The primary endpoint was pathological complete response (pCR) rate, and secondary endpoints included progression-free survival (PFS) and overall survival (OS).

The results showed that the pCR rate was 37% in the Opdivo + chemotherapy group compared to 7% in the chemotherapy group. The surgical resection rate was 93% in the Opdivo + chemotherapy group versus 69% in the chemotherapy group.

For secondary endpoints, the 24-month PFS rate was 67.2% in the Opdivo + chemotherapy group compared to 40.9% in the chemotherapy group. The 24-month OS rate was 85% in the Opdivo + chemotherapy group versus 63.6% in the chemotherapy group.

Regarding safety, the incidence of Grade 3-4 adverse events was 19% in the Opdivo + chemotherapy group and 10% in the chemotherapy group.

Based on the Phase 2 results, Mariano Provencio stated that neoadjuvant Opdivo plus platinum-based chemotherapy improved pCR rate, PFS, and OS in patients with Stage IIIA/IIIB NSCLC.
