(Ovarian Cancer) Is Rucaparib Effective as Maintenance Therapy?

On June 4, 2022, Bradley J. Monk and colleagues at Piper Cancer Care Network published results in the Journal of Clinical Oncology from the ATHENA–MONO/GOG-3020/ENGOT-ov45 (NCT03522246) Phase III clinical trial evaluating rucaparib as maintenance therapy in platinum-sensitive stage III/IV ovarian cancer patients after surgical resection.

The ATHENA–MONO/GOG-3020/ENGOT-ov45 Phase III trial was an international multicenter study that randomized platinum-sensitive stage III/IV ovarian cancer patients after surgical resection (N=538) in a 4:1 ratio to:

• Rucaparib group: Rucaparib 600 mg twice daily (N=427)
• Placebo group (N=111)

The primary endpoint was progression-free survival (PFS).

Results:

Among HRD patients (rucaparib group N=185, placebo group N=49):

• Median PFS: Rucaparib group 28.7 months vs. placebo group 11.3 months

Among non-HRD patients:

• Median PFS: Rucaparib group 12.1 months vs. placebo group 9.1 months

Among all patients:

• Median PFS: Rucaparib group 20.2 months vs. placebo group 9.2 months

Safety:

Grade ≥3 adverse events confirmed in the majority of patients were:

• Anemia: Rucaparib group 28.7% vs. placebo group 0%
• Neutropenia: Rucaparib group 14.6% vs. placebo group 0.9%

Based on the ATHENA–MONO/GOG-3020/ENGOT-ov45 trial results, Bradley J. Monk and colleagues concluded: Rucaparib maintenance therapy in platinum-sensitive stage III/IV ovarian cancer patients after surgical resection demonstrated good anti-tumor efficacy regardless of HRD status.
