On November 26, 2021, Dr. Junko Tanizaki and colleagues from Kindai University Faculty of Medicine published results in Annals of Oncology from the NivoCUP Phase 2 clinical trial evaluating the efficacy and safety of the anti-PD-1 antibody Opdivo (nivolumab) in cancer of unknown primary (CUP) patients.
This multi-center, single-arm Phase 2 trial administered Opdivo (240 mg every 2 weeks, up to 52 cycles) to CUP patients (N=56), with objective response rate (ORR) as the primary endpoint. Of these patients, 45 had received one prior line of therapy.
The trial was initiated because CUP carries a poor prognosis, and immune checkpoint inhibitors have demonstrated efficacy across multiple cancer types. The study aimed to evaluate the therapeutic value of nivolumab in this difficult-to-treat population.
Results (in patients with prior treatment, N=45):
- Objective response rate (ORR): 22.2%
- Median progression-free survival (PFS): 4.0 months
- Median overall survival (OS): 15.9 months
Safety: The safety profile was consistent with existing nivolumab clinical trial data. No treatment-related deaths were reported.
Conclusion: Based on the NivoCUP trial results, Dr. Tanizaki and colleagues concluded that Opdivo demonstrated promising anti-tumor activity in CUP patients, potentially representing a new treatment option for this population.
