From February 16–18, 2023, Neeraj Agarwal of Huntsman Cancer Institute and colleagues presented at ASCO GU 2023 in San Francisco, California, the efficacy and safety results of Talazoparib + Enzalutamide as first-line treatment in metastatic castration-resistant prostate cancer patients in the TALAPRO-2 Phase 3 clinical trial.
The TALAPRO-2 Phase 3 clinical trial was a randomized, double-blind trial that randomized metastatic castration-resistant prostate cancer patients (N=805) to either Talazoparib (once daily, 0.5 mg) + Enzalutamide (160 mg) group (N=402) or placebo group (N=403). The primary endpoint was radiographic progression-free survival, and secondary endpoints were overall survival, objective response rate, and PSA index.
The results showed that the median radiographic progression-free survival was: Talazoparib + Enzalutamide group: not reached / placebo group: 21.9 months. HRR-deficient patients in the Talazoparib + Enzalutamide group showed a 54% reduction in risk of radiographic progression or death compared to the placebo group. Patients without HRR deficiency in the Talazoparib + Enzalutamide group showed a 30% reduction in risk, while secondary endpoint data were not yet mature.
Regarding safety, the incidence of Grade 3–4 adverse events was: Talazoparib + Enzalutamide group 71.9% / placebo group 40.6%. Grade 3 or higher adverse events observed between groups were: Talazoparib + Enzalutamide group — anemia, neutropenia, and thrombocytopenia; placebo group — hypertension, anemia, and fatigue.
Based on the TALAPRO-2 trial results, Talazoparib + Enzalutamide as first-line treatment in metastatic castration-resistant prostate cancer patients improved radiographic progression-free survival regardless of HRR deficiency status.
