Enhertu (T-DXd) Superior to Choice of Therapy in HER2-Positive Breast Cancer: DESTINY-Breast02 Trial

On August 17, 2022, Daiichi Sankyo and AstraZeneca announced positive topline results from the DESTINY-Breast02 Phase 3 clinical trial. This study evaluated the efficacy and safety of Enhertu (Trastuzumab deruxtecan) in patients with HER2-positive unresectable and/or metastatic breast cancer who had been previously treated with T-DM1 (Kadcyla).
Study Design: DESTINY-Breast02 Trial
DESTINY-Breast02 is a global, randomized, multicenter trial designed to confirm the results seen in the earlier DESTINY-Breast01 trial. Patients were randomized to receive either:
- Enhertu (T-DXd).
- Investigator’s Choice of Treatment (either Trastuzumab/Capecitabine or Lapatinib/Capecitabine).
The primary endpoint was progression-free survival (PFS) as assessed by independent central review.
Key Findings
The trial achieved its primary and key secondary endpoints:
- Progression-Free Survival (PFS): Enhertu demonstrated a statistically significant and clinically meaningful improvement in PFS compared to the investigator’s choice of treatment.
- Overall Survival (OS): The trial also confirmed a significant improvement in overall survival for patients treated with Enhertu.
- Safety: The safety profile of Enhertu was consistent with previous clinical trials. While interstitial lung disease (ILD) occurred, its incidence and severity were in line with existing data.
Conclusion
The DESTINY-Breast02 trial reinforces Enhertu’s position as a transformative therapy for HER2-positive metastatic breast cancer patients who have progressed after T-DM1. These results support the continued expansion of access to Enhertu globally, providing a superior treatment option over traditional chemotherapy-based combinations.
